Use these forms to claim back eligible medical expenses from your DCare insurance plan. You are required to use one form per insured person and per condition. Claim form should be filled out and submitted with all necessary documentation; Receipts, Doctor Referrals and Results of Diagnostic Tests/Treatment.
Claims should be submitted either by email to dcare@healthwatch.gr, or through our application (App) for mobile devices. You can download it from Google Play (Android) or the App Store (iOS).

Search for the word `exelixis` in the Play Store environment.
Locate Exelixis Software and click on `EXELIXIS SOFTWARE` in blue letters.
Some options will appear; locate Health Watch and click Open.
